LIABILITY/INJURY WAIVER AND RELEASE:
In consideration of being allowed to participate in any way in the St. Ursula Academy Summer Camp related events and activities the undersigned acknowledges, appreciates and agrees: 1) the risk of injury from the activities involved in this camp could be substantial and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation; 3) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the person(s) leading the camp or activity immediately; and, 4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless all those associated with the St. Ursula Academy Summer Camps, their officers, officials, agents, and other employees, other participants, sponsoring agencies, sponsors, advertisers and if applicable, owners and lessors of premises used to conduct the event ("Releasees"), with respect to all and any injury, disability, death or loss or damage to person or property, whether arising from negligence of the releasees or otherwise, to the fullest extent permitted by law.
This is to certify that I, as a parent/guardian participant with legal responsibility for this participant, do consent and agree to her/my own release as provided above, of all the releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless and release from any and all liabilities to my minor child's/my own involvement or participation in this program as provided above, even if arising from their negligence, to the fullest extent permitted by law.
Permission and authorization is hereby granted to person(s) leading the camp for my child to receive emergency medical treatment, if needed, and I certify that there are no limits or restrictions to my child's participation in the camp except as stated in writing in this waiver.
Photo Release: As the parent or guardian of the minor named above, I hereby grant St. Ursula Academy permission to use my child's likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications and social media.
